Family Input

(upbeat music) (electronic whooshing) - Just like we get input from our students to help write the IEPs and plan for instruction, we get input from our families as well. This is a general sheet that collects information about students' family and home. It also gathers the family's opinions about the students' preferences. Many of our students have very specific sensory preferences, things they love, or things they really try to avoid. This form helps families let us know about the things students like or don't like for each of the five senses. We try to incorporate a lot of cross-curricular hands-on activities and real world learning in the classroom. So we cook frequently. This form gathers information about students' food preferences and allergies, so we can use that in our planning. We wanna make sure that information from the family is included in the IEP, so we have them fill out this questionnaire, which can also be done as an interview. It asks them about their goals for the student, any concerns they may have and their vision for the student's future. (upbeat music)
